PRIVACY POLICY - EPIPHANY COUNSELLING
Shelley Harrison / Epiphany Counselling is registered with the Information Commissioners Office as a Data Controller and will continue to be. ICO Registration Number – ZA549967. The sensitive personal information that I hold is known as ‘special category data’.
Personal Information that I will collect:
• Name
• Address
• Email Address
• Contact Number
• Presenting Issue / Area of focus
• Summary of session
How I will store your Personal Information:
• Paper: Written notes (described below), using a ‘client code’ to protect confidentiality. Notes are stored in a secure locked filing cabinet of which only I have access to.
• Smartphone: Storage of Name and Contact Number. My phone is password protected and is stored in my locked filing cabinet when not in use.
• Email: Your email address and our correspondence will be stored in my Gmail email account. The devises that I access email correspondence from are my laptop and smartphone, both of which are password protected.
• Laptop: A client and supervision log is kept in excel worksheet format. The folder which the file is saved into and the laptop itself are both password protected. My laptop has current AVG antivirus protection.

• Counselling Contract
• Consent Form
• Brief Session Notes
• Client Code (for linking documents)
Electronic documents held:
• Contact Name and Telephone Number
• Email correspondence
• Excel client log
• Excel supervision log
• Payments made Electronically through Bank Transfer or ‘Sum Up’ Payment Devise - Your Account Name and details may appear on my Bank Account Records. If payment is made via the Sum Up Payment Devise, your Account Name and details may also appear on the Sum Up App that assists in the payment process.
How I may share your Personal Information:
• Supervision – On a monthly basis I meet with a Supervisor. On occasion I may bring content from our work together to reflect on with my Supervisor to assist in my best practice, you would be kept anonymous or referred to by your first initial only.
• Emergencies - If your health is in jeopardy (provided I have your consent) I may share your contact information with an emergency healthcare service (eg. Mental Health Crisis Team).
• If I have become aware of your intent to cause harm to another person / organization (eg. Child Protection, Terrorism) the Law may require that I inform an authority without seeking your permission. In such a situation, the law may require that I share your personal information without your knowledge (known as: whistle blowing).
Erasing your Information:
•
Electronic / Smartphone Correspondence - When we have finished working together, I will erase electronic copies of your information and correspondence within one month (eg. Email correspondence, text messages, mobile contact).

Paper Documents - I will securely store within my lockable filing cabinet any paper documents in relation to you or our work together (eg. Counselling Contract, Consent Form, Brief Session Notes) I will keep this information for three years following our last session. Information is kept for this period of time so that I have a reference of our work for situations such as your returning to counselling in the future. After the three years have passed I will shred the written paper documents.
Your Rights:
Unless subject to exemption under the GDPR, You have the following rights…
• To be informed what information I hold (ie this document).
• To see the information that I hold about you.
• To rectify any inaccurate or incomplete personal information.
• To withdraw consent to me using your personal information.
• To request your personal information to be erased when it is no longer needed.
• To lodge a complaint with the Information Commissioners Office.
